?DIRECT DEPOSIT AUTHORIZATION:

I hereby authorize Universal Land Construction (Employer), either directly or through its payroll service provider, to deposit any amounts owed to me, by initiating credit entries to my account held with the financial institution of my choice, as identified. In the event that the Employer or its payroll service provider deposits funds erroneously into my account, I authorize the Employer or its payroll service provider to debit my account for an amount not to exceed the original amount of the erroneous credit. I acknowledge that the origin of ACH transactions to my account must comply with the provisions of US law.

I understand that I have the right to refuse consent or revoke authorization of direct deposits at any time, and this authorization shall remain in effect until the Employer has received my written notice of its termination 5 business days prior to the date in which funds are deposited in my account.
